Referral Process + Contact Information

Thank you for your interest in our services. To schedule an appointment with Dr. Yip, we request that you obtain a referral from your healthcare provider, such as your family doctor, nurse practitioner, or another specialist.

*Please Note: We cannot schedule appointments without a referral.

Referral Process

Where to Submit Your Referral

Please ensure that the referral is faxed to (403) 288-6992.

Once received, all referrals and imaging reports are reviewed by Dr. Yip, and are triaged appropriately. Please note: sinus x-rays will not be accepted as they are not sensitive or specific for sinonasal findings.

Criteria for Referrals

*Please note that Dr. Yip does not perform functional or cosmetic rhinoplasties.

> 8-12 weeks (ex. nasal congestion, nasal blockage, hyposmia / reduced sense of smell, facial pain/pressure, post-nasal drip, and/or nasal discharge): a trial of adequate medical therapy for at least 2-3 consecutive months must been performed and a recent CT report (within the past 12 months).

Trial of topical intra-nasal corticosteroid spray BID +/- intra-nasal antihistamine spray BID +/- intra-nasal ipratropium bromide spray for at least 3 consecutive months.

A recent CT and/or MRI report (within the past 12 months).

A recent CT and/or MRI report (within the past 12 months).

A recent CT and/or MRI report +/- beta-2-transferrin testing of nasal discharge (at least 5 drops).

A recent CT report that was performed AFTER the last surgery along with prior notes from surgeon (if available).

A referral from your healthcare provider is required for individuals with epistaxis if nasal moisturization or lubrication fails to control the nosebleeds after 3-4 weeks of consistent use, or if they experience severe or recurrent nosebleeds. An accompanying CT sinus may be required in certain circumstances. This may indicate an underlying issue that requires further evaluation and specialized treatment by an ENT specialist.
