Forms & Fees
- Sick note/clinic visit notes are provided at no charge, and can be obtained following your appointment at the front desk.
- Insurance/Short Term Disability forms – $100-150
- Please be aware there is a $200 cancellation fee for patients that cancel or no show. To rebook your appointment this fee must be paid before rebooking occurs. Please contact us at (825) 540-3545 if you are unable to attend your appointment.
Referrals & Appointments
- A referral from a Primary Care Provider (GP or NP) is required for all appointments covered by Alberta Health
- Once your referring physician sends in a new referral, we will attempt to contact you twice. On the third attempt if there is still no answer, and there has been no attempt to contact us back, we will send the referral back to your referring physician. You will come off our waitlist at that time.
- You will get a text and email notification reminding you of your appointments.
Surgical Bookings & Cancellation List
- Once you have signed a surgical consent with Dr. Yip, you are put on the surgical waitlist. You must then notify your employer, so when we with call with a proposed date, you are able to confirm. You can then inform your employer of your procedure date.
- Depending on the signed procedure, we will explain the time frames before leaving the office. You can have the choice of going on the cancellation list.
- If you go on the cancellation list, you can say no to the date we offer once. After that you will go back on to the normal surgery list without losing your spot.
- Being on the cancellation list also means you must be available within two days to a week’s notice.
- When being called from the cancellation list we will not offer another date, this is only offering the date for which we have a cancellation.
